To date, there is not sufficient documented communication between the chronic clinic and the acute setting. Crucial dialysis processes and patient information are unfortunately not captured and easily transmitted in a timely way. This creates a multitude of challenges for the staff in the acute setting.
Information regarding: the fistula care, the buttonhole, the catheter access and how it is flowing, are just a few of the many pieces of information that are vital in order to provide essential patient dialysis care.
Visions For Care has developed communication documents and methods that are easy to use between the hospital and chronic clinic setting. The time and discomfort for the patient is dramatically decreased and the process is much more efficient for the attending nursing staff.
